You can then print and fill out your forms to bring to your first visit. This will help us process your information and get you seen as quickly as possible.

What to Bring

Please arrive early so we can finalize any paperwork and remember to bring:

• Your insurance card

• Your medical and surgical history

• A list of your current medications (over-the-counter, too) including dose and frequency

• Any recent test results, X-rays, or relevant records
